You may ship the ashes to Hawaii yourself, or you may ask your funeral service provider to assist you.

If you would like we can send you the necessary forms and information via email, fax, or regular postal mail.  

1. Please wrap the container with the cremated remains with a padded material and place all in an appropriate shipping box.   

2. Mark the outside of the shipping box on the address side with "Cremated Remains".

3. Include the following additional forms & information inside the shipping box, or attach it securely to the outside of the shipping box::

a.  Ash Scattering Authorization Form - click here to print form - 

b.  Disposition Papers (burial permit) from funeral provider ( this shows when the body was cremated). You can get this from the funeral director or the crematorium.  This document provides proof that the body was legally cremated.

c. Copy of the Death Certificate

d. Any special requests

e. Payment Form ( if paying by credit card) - click here to print form -
